Implementing Effective Care Coordination Models for Advanced Illness Patients

With support from PCORI as part of their Eugene Washington Engagementfor Capacity Building Award Program, C-TAC is convening a Regional Research Forum so that health system leaders, individual providers, payers, and community and state coalitions can learn about four pioneering PCORI-funded research studies that present innovative models of care for patients living with an advanced illness. The aim of the Forum is to connect the PCORI researchers with advanced illness care implementers to improve the quality of life of advanced illness patients and their families.
The purpose of this Forum is to place these person-centered, actionable PCORI studies in the hands of implementers – health system leaders, individual providers, payers, community and state coalitions, and advocacy groups – who can use the research findings to improve the delivery of care for patients living with advanced illness.
The following PCORI-funded investigators will present their research:
