C-TAC Member Spotlight: Roots & Roads Community Hospice Foundation

Roots & Roads Community Hospice Foundation (formerly Casa de la Luz Foundation) was established in 2000 to transform the end-of-life experience into the final act of living well for those on hospice care in our community.  For the past 21 years, we have provided financial support to patients and families, end-of-life education to the community and research grants to improve EOL care.

In June 2021, the Foundation transitioned to become Roots & Roads Community Hospice Foundation.  Our rebrand allows us to reach more hospice patients by expanding our focus to a larger area of Southeastern Arizona (Pima, Pinal, Cochise and Santa Cruz counties).

Highlights from 2020 include:

  • 98 hospice patients and their families received more than $70,000 worth of financial support for things like housing, transportation, and final arrangements.
  • 97 people participated in bereavement groups and received Foundation-sponsored grief workbooks.
  • 235 individuals attended our annual End-of-Life Conference, a unique, day-long educational event. 
  • 108 Veterans were honored for their service through Foundation-sponsored ceremonies.

As the elderly population skyrockets over the next few decades, our support of patients at end-of-life, and their families, will be even more critical.  Housing for those no longer able to be cared for at home, but not “sick enough” to receive funded inpatient hospice care, is one of the greatest needs we address.  We recognize that critical systems change is needed to best serve those in our community.  C-TAC is a phenomenal convening organization which can move the systems change conversation forward in a very tangible way.  My recent participation in C-TAC’s incubator: “Integrating Needed Services & Supports Into Existing Care Models” is a great example of such a convening.  We are excited to work with C-TAC staff and partnering organizations to have these conversations and spur the changes that will result in a more equitable society as we transform the end-of-life experience into the final act of living well.
