Eternally is here to radically improve end-of-life care. This is something that impacts everyone in the world, yet we are still using bizarrely antiquated methods of planning and preparing for it. Most importantly, most Americans feel they don’t have the access to someone who can help them with planning. Eternally acknowledges that this is a societal problem that needs to be remedied. Our co-founder Matti Perilstein states that Eternally is combating this issue because “we [Eternally] are focused on moving the needle forward and making a difference for millions of people around the country who would otherwise not have access to end-of-life resources or advanced care planning”.

Eternally’s one-on-one patient conversations are facilitated by expert clinicians who help patients complete their living will/advance directive from the comfort of their own home (via phone call or telehealth video); driving our mission to enable this support for everyone, anywhere, at any time. Eternally partners with hospital systems, long term care facilities, non-profits, and specialty organizations to provide their patients or members access to Eternally’s personalized Advance Care Planning resources. Individuals who visit https://www.myeternally.com/ can also have this conversation should they want to connect with a clinician and complete their Advance Directive with the support of a clinician. Eternally is an incredible resource for individuals to speak one-on-one with a clinician in a relaxed setting. This conversation takes time and we understand if you need to meet with us more than once. Our use of telehealth technology ensures that everyone has access to us no matter where they are, and we personalize the conversation, the mode in which we meet, and work with you until you are happy with the results. We are your partner in this, and we are always here to help you, now and in the future.

Eternally’s success can be attributed to our certified Care Team made up of PAs, NPs, MSNs, RNs, MSWs, LCSWs, and end-of-life doulas with an average of 18 years of experience. The Advance Care Planning process can be emotional and overwhelming, but our Care Team Members guide the patients through the process, easing their concerns and fear surrounding this process.
