The Blog to Transform Advanced Care
Advancing Care through Innovation, Observation and Collaboration.
Note: Due to a technical issue, the first 1-2 minutes of the webinar is not captured in this recording. The video begins during introductory remarks by our moderator, C-TAC Policy & Advocacy Manager Davis Baird. The COVID-19 pandemic has highlighted the importance and urgency of advance care planning (ACP) and ensuring patients’ goals and values inform the care they receive. While there is strong interest among Medicare Advantage (MA) plans to address this urgency, it remains challenging to design ACP strategies and programs that work well and make a meaningful difference for members, providers, and plans. With new federal policies that support MA plans’ ACP efforts, such as the requirement that all participants in the Value-Based Insurance Design (VBID) demonstration implement a broad Wellness and Health Planning (WHP) strategy, the role plans can play in developing increasingly effective solutions that help members and their families identify and record their treatment goals and wishes is more important than ever.
On this 75-minute webinar, leaders from CMMI and two innovative VBID-participating integrated delivery systems will discuss the value of high-quality ACP/WHP and highlight strategies that have successfully engaged members and networked clinicians. Hosted by C-TAC, the webinar will focus on actionable steps plans can take to accelerate progress on their own ACP programs.
