Vital Decisions’ mission is to be the force, the catalyst in this country, to fundamentally change how individuals are cared for during an advanced illness. Since 2008 we’ve been steadfast in our mission by partnering with health plans and other entities to provide our suite of Advance Care Planning solutions free to their members. We connect with individuals and their families by phone and through video sessions allowing us to reach the outermost edges and most vulnerable members of our nation. By offering our services via phone we eliminate equity barriers that patients often face – transportation, access, racialization and the emotional barriers that often come with this type of work.
We work towards activating members in Advance Care Planning conversations with their loved ones and medical team, provide information regarding medical crisis decision making, assist with completing appropriate documentation, and can connect members with additional resources, such as palliative care when applicable.
Earlier this year we extended one of our solutions – My Living Voice – to the public, free of charge. Now, more than ever, there is a need for loved ones and doctors to know one’s wishes for medical care in case of a crisis. So, beginning with this year’s National Healthcare Decisions Day in April, we made My Living Voice available. The online platform is an easy to use and secure digital tool that allows an individual to complete their own advance care plan, choose a healthcare proxy, and share their completed, state specific, documentation with those who matter most to you, all within a matter of minutes.
We’re excited to support C-TAC and be a sponsor at this year’s summit because of our aligned missions and drive towards change in how Advance Care Planning is delivered to the most vulnerable individuals. Despite the change of venue of this year’s summit compared to the last 2 years Vital Decisions has sponsored, we believe this year’s summit may be the most vital one yet. Supporting the most vulnerable Americans is not new to us, they’ve been our driving force all these years, but the COVID-19 pandemic has now highlighted their insecurities and needs to a more captive and broader audience. C-TAC and its members have the opportunity to come together this year and mobilize even more intently than before, growing closer towards connecting individuals facing advanced illness with value alignment, dignity, and the tools for self-advocacy.
