C-TAC is one of seven organizations participating in a new initiative to improve care for older adults and those facing serious illness and the end of life. The effort will be co-funded by the Cambia Health Foundation and the John A. Hartford Foundation, in a new partnership announced on Aug. 29.

Other organizations participating in the three-year project include Ariadne Labs/Serious Illness Care Program, the Center to Advance Palliative Care (CAPC), Respecting ChoicesThe Conversation Project, the National POLST Paradigm, and VitalTalk. Subcontracted organizations include the American Academy of Hospice and Palliative Medicine (AAHPM), the National Coalition for Hospice and Palliative Care (The Coalition) and the National Hospice and Palliative Care Organization (NHPCO).

C-TAC and other participating organizations will work with the Cambia Palliative Care Center of Excellence at the University of Washington to spread public messaging through a variety of channels.
