A new Health Affairs article investigates the new payment models announced in April by CMS, Primary Care First and Direct Contracting. The authors covers issues ranging from implementation of the new models to quality measurement.
The article also includes an examination of the Serious Illness Population track of Primary Care First, citing the influence of C-TAC on the model, as well as the American Academy of Hospice and Palliative Medicine.
“Care for high-risk, high-need patients earns significant attention, especially in the Serious Illness Populations (SIP) track within PCF. This model specifically builds on proposals from the American Association for Hospice and Palliative Medicine (AAHPM) and the Coalition to Transform Advanced Care (C-TAC) and offers the clearest glimpse yet into how CMS is incorporating new delivery models targeting this population,” the authors wrote.
