The Blog to Transform Advanced Care

Advancing Care through Innovation, Observation and Collaboration.

We are excited to announce that C-TAC’s 2018 Annual Report is now available for download. It reflects on a milestone year in the movement to transform advanced care. Highlights from the report include:

  • In 2018, U.S. Department of Health and Human Services’ Physician-Focused Technical Advisory Committee (PTAC) unanimously recommended our Advanced Care Model for testing. In a public letter written in June, HHS Secretary Alex Azar expressed particular interest in the C-TAC model.
  • We expanded our efforts to support caregivers by collaborating with Driving Miss Norma authors and inaugural C-TAC Caregiver Fellows Tim Bauerschmidt and Ramie Liddle, who are contributing blog posts on caregiving and connecting online communities with tools and information. With support from the Cigna Foundation, we also undertook caregiving pilot projects aimed at sharing caregiving resources in faith communities.
  • Building on the success of our national Advanced Care Transformation IndexSM, we have developed indices to measure advanced care quality on the state level. The new index will offer critical insights on care quality to help states improve the care they deliver to the advanced illness population.
  • Stepping outside of the Beltway, we held the 2018 National Summit in Denver, CO. The event was a resounding success, convening 400 healthcare leaders to share the latest developments in advanced care and discuss innovative ideas to advance the movement.

C-TAC is proud of the progress we have made in the last year, with the support of our partners, members and sponsors. We invite you to take a look back at this groundbreaking year for the advanced care movement.
