Member Spotlight: MedStar House Call Program

The MedStar House Call Program was founded in 1999 by Drs. Eric De Jonge and George Taler to serve high-cost and high-need patients in their homes. The program model is designed to offer support around the clock, with the goal of allowing people to remain at home throughout all stages of their illnesses.

Today, the program is a nationally recognized service that offers patients who have difficulty getting to the doctor’s office high-quality at-home care, as well as an inpatient team that cares for House Call patients in need of acute or emergency hospital care. Moreover, the program also boasts social services that link people and their families – as well as their caregivers – to much-needed community resources, such as support groups and even telephonic meditation classes.

Today, the program is a nationally recognized service that offers patients who have difficulty getting to the doctor’s office high-quality home-based primary care, as well as an inpatient team that cares for House Call patients in need of acute or emergency hospital care. The program also boasts social services that link people and their family caregivers to much-needed community resources, such as home health aides, support groups and even telephonic meditation classes.

Physicians and nurse practitioners make routine and urgent house calls on weekdays and are available by phone at all times. They can deliver a full range diagnostic tests and treatments at home that are comparable to an urgent care center. Supported by a mobile electronic health record, the House Call team coordinates home nursing, physical therapy, and hospice services, and allows patients to retain their previous specialists.  In DC, the program offers social work case management to those with the Medicaid Elderly and Persons with Disability (EPD) waiver.

“There was always someone from the House Call Program to guide us on what to do for our parents,” said the daughter of a house call patient. “They got to stay at home in the comfort of their home, and never worried about going to a Nursing Home.”
