C-TAC Awarded a Grant to Develop a Serious Illness Care Model Implementation Framework

March 2, 2017 – C-TAC is proud to announce that funding has been received to develop an implementation framework for a serious illness care model that provides high-quality, community-based care for those with serious illness.

C-TAC will act as a national convener of health care executives and serious illness care program experts, to be held at the Petrie-Flom Center at Harvard Law.  C-TAC will collaborate with an advisory group including the University of California Davis School of Nursing, the Center to Advance Palliative Care and consultants to the Gordon and Betty Moore Foundation. The project begins with a review of existing thinking and frameworks related to serious illness care to develop and refine a blueprint that outlines core care model implementation concepts.

C-TAC will also identify various care model interventions for literature review and conduct convenings with the advisory group, healthcare leaders, and policymakers for final review and refinement before dissemination. The framework will be completed by summer of 2017.
