C-TAC Collaborates with JAHF and Industry Stakeholders to Transform Advanced Illness Care
June 19, 2015 – C-TAC is honored to be one of seven innovators convened at a meeting at the John A. Hartford Foundation (JAHF) office in New York focused on “End of Life and Serious Illness,” gathering leading innovators in the field and funders with deep interest in this area.
C-TAC praises JAHF’s leadership on this issue and is excited to participate in this collaboration with the JAHF and is an important step to delivering high-quality, person-centered care and ensuring that all those with advanced illness, especially the sickest and most vulnerable, will receive comprehensive, high-quality, person- and family-centered care that is consistent with their goals and values and honors their dignity.
