The first multi-provider collaborative for those with advanced illness in Florida has been launched in consultation with C-TAC Innovations in the St. Augustine area. Flagler Hospital, First Coast Health Alliance, and Alivia Care announced the Flagler Advanced Care Services (FACS) program, designed to improve the quality of care for people with serious and late-stage chronic illnesses while honoring their values and personal care goals.
Patients referred to the program benefit from improved support services in their preferred care settings, an interdisciplinary care team that addresses physical, psychological, and spiritual needs, and nurse-led in-person and telephonic clinical support at every step of care. Moreover, there are no out-of-pocket costs for eligible patients. This care delivery model allows the patient to reclaim control of their treatment and enjoy a higher quality of life in their own home.
As the first and only such collaborative between a hospital and insurance company in Northeast Florida, FACS represents a new standard of care delivery and promises to be an example for similar programs across the country. Learn more here.
